Today’s health care system is quite complex. In order to assist you with your health insurance all our insurance forms are submitted by our computerized billing department. This allows our office to participate with most insurance companies and do all the billing electronically. We work hard to participate with all insurance companies and follow their procedures so that your claim will be paid in a prompt and efficient manner. However the qualifications and limits of your coverage unfortunately do change frequently and we try to stay current. Please call your insurance company to verify your coverage and if you have further questions, please call our billing office at 614-890-7224 for assistance. In order for us to submit your charges for your visit, you will need to bring the following information when you come to your appointment:

1. Your current insurance card
2. A referral form if required by your insurance- please call them if you are not sure. HMO patients must have and are responsible for obtaining their own referrals from their primary care physician before your visit.
3. Any required co-payment or deductible payment
4. A list of your current medications
5. Completed New Patient Forms

We realize your time is valuable and we make every effort to see our patients on time. If you are unable to keep your appointment or are running late, we ask for as much advance notice as possible.
